Do I Qualify for Assistance?



You have reached the eligibility quick-test for the ESP International Charitable Foundation Employee Assistance Fund.If you answer "yes" toall of the following questions, please complete the application documentation in order to be considered for a grant from the Fund. If you answer "no" to any of the questions, youdo not qualify for assistance.


Establishing eligibility can, in some situations, be complicated. This quick-test has been designed to assist you with this pre-qualification process.



Question:Are you currently employed by ESP Internationalin a position regularly scheduled to work 20 or more hours per week, and have you been employed by ESP International for at least twelve months?(Employees onapproved leave of absence who otherwise qualify may answer "yes.")


Question:Have you experienced a qualifying event in the last four months?Qualifying events include a death in the immediate family (employee, spouse/domestic partneror child), a serious or life threatening illness or injury in the immediate family, a natural disaster (such as tornado, earthquake, wild fire, etc.), or another catastrophic or extreme circumstance (such as a violent crime against you or a family member).


Question:Do you have evidence of this event that you can provide with your application, such as an obituary, newspaper report, police or fire report, or physician's letter?


Question:Has the qualifying event caused a serious disruption of your financial security, resulting in possible eviction, utility cancellation, or an inability to provide for basic family needs such as food and clothing?



Question:Do you have past due bills or letters from utilities, vendors or a landlord that demonstrates the amount owed?
